In the absence of a comprehensive and multifaceted summary of health issues related to the ketogenic diet (KD), we gathered evidence on the use of the ketogenic diet regarding its effects on the microbiome, epigenome, diabetes, loss of ‘ weight, cardiovascular health and cancer. . A KD diet can increase the diversity of microbes and increase the proportion of Bacteroidetes and Firmicutes. The epigenome is susceptible to KD as it produces a signaling molecule known as hyd-hydroxybutyrate (BHB). KD helped diabetic patients lower HbA1c and lower insulin levels. There is evidence that KD can help with weight loss, better mood, and appetite control. Evidence also suggests that eating a high-fat diet improves lipid profiles by reducing low-density lipoproteins (LDL), increasing high-density lipoproteins (HDL) and reducing triglycerides (TG ). Because of the Warburg effect, KD is used as an anti-cancer drug, making it more susceptible to chemotherapy and radiation. Ketogenic Low‐cho, High‐fat Diet: The Future Of Elite Endurance Sport?

The Ketogenic diet has begun to grow in popularity as doctors and researchers investigate its potential benefits. Nutritional ketosis, the end of the ketogenic diet, is achieved by stopping carbohydrate intake, reducing protein, and increasing calories from fat [1]. In fact, this carbohydrate restriction causes the body to switch from glucose metabolism as the primary source of energy. This results in the use of ketone bodies from fat metabolism, a metabolic state in which the body chooses to use fat as the primary source of fuel. Recent studies using low-carbohydrate, high-fat (LCHF) diets, such as the ketogenic diet, show promise in helping patients lose weight, improve symptoms of metabolic syndrome, reduce or eliminate the need for insulin in patients with type 2 diabetes. II [2], reduce inflammation, improve epigenetic profiles, modify the microbiome, improve lipid profiles, improve anti-inflammatory drugs cancer, and increases longevity [3] and brain function.

The number of Americans suffering from obesity, diabetes, and metabolic syndrome is increasing. Symptoms of metabolic syndrome include obesity, insulin resistance, elevated triglycerides, and hypertension [4, 5]. All these health symptoms increase the risk of heart disease, diabetes, obesity, and Alzheimer’s disease. According to WebMD, there are currently 27 million people with type 2 diabetes and 86 million with prediabetes. In addition, the Centers for Disease Control and Prevention (CDC) also estimate that 40% of adults and 20% of American children are obese [6, 7]. Many researchers believe that these diseases are caused by carbohydrate intolerance and insulin resistance. Therefore, a diet that reduces exposure to carbohydrates, including whole grains, may be a reasonable strategy to improve health [8]. Accordingly, two dietary models, the standard ketogenic diet, and the medical ketogenic diet (Figure 1), which limit carbohydrate intake to varying degrees are being studied for their health benefits. The ketogenic diet, which is very low in carbohydrates and proteins, is used to treat epilepsy and cancer. However, the Dietary Guidelines for Americans suggest that between 45 and 65% of calories should come from carbohydrates (Figure 1). If a person consumes 2000 calories per day, this is equivalent to 225-325 g of carbohydrates per day [9].

One diet that seems to be becoming mainstream is the high carb/high fat diet. However, there is a difference between a low carb and low ketogenic diet (LCKD). Ketosis is usually achieved by fasting or cutting out carbohydrates. It is important to explain that a low-carbohydrate diet refers to a diet that contains 50 to 150 g of carbohydrates per day. However, even though this is lower in carbohydrates than the average American diet, it is not low enough to enter ketosis. It is only when the patient restricts carbohydrates to less than 50 g/day that the body is unable to fill the body with glucose and turn it into fat burning [10]. The ketogenic diet is a modification of the food pyramid supported by the Dietary Guidelines. Therefore, instead of eating a diet high in carbohydrates, it is high in fat (Figure 2). Carbohydrate restriction lowers blood glucose levels, and subsequent changes in insulin will instruct the body to switch from fat storage mode to fat oxidation mode [10] . Once fat is used as the primary fuel source in the liver, the body’s production of ketones begins, a process known as ketogenesis. During ketosis, three main ketone bodies are produced and used by the body for energy: acetone, acetoacetate and hyd-hydroxybutyrate [11]. All cells that contain mitochondria can supplement their energy with ketone bodies, including the brain and muscles. In addition, research suggests that β-hydroxybutyrate acts as a signaling molecule and may play a role in appetite suppression.

However, there are differences in the type of data available. Therefore, the purpose of this review is to show the role of the ketogenic diet in the microbiome, epigenetics, weight loss, diabetes, cardiovascular disease and cancer as mentioned above (Figure 3).

Their initial results were confirmed by a Mayo Clinic study with a different population [13]. [15], the average heritability of the gut microbiota is only 1.9%, while more than 20% of the variation was related to diet and lifestyle.
