The 2015 Dietary Guidelines for Americans recommends reducing cholesterol intake. However, dietary cholesterol and its sources have not been well characterized. We assessed dietary cholesterol by age, sex, race, and food sources using 24-hour dietary recall data from a nationally representative sample of 5047 adults aged 20 years or older who participated in NHANES (months 2013–2014). We also reported trends in cholesterol intake across seven NHANES studies. Average dietary cholesterol was 293 mg/day (348 mg/day for men and 242 mg/day for women) in the 2013-2014 study; 39% of adults had cholesterol over 300 mg per day (46% of men and 28% of women). Meat, eggs, whole grains and dairy products are the main sources of dietary cholesterol, accounting for 96% of total dietary cholesterol. Cholesterol and food source estimates differed by age, sex, and race (p < 0.05 each). The mean cholesterol level in the general population was constant at ~290 mg/day from 2001-2002 to 2013-2014 (p-trend = 0.98). Results should inform public health efforts to establish dietary guidelines and adjust dietary recommendations.

Although all cholesterol comes from animal sources (meat, milk, and eggs), many foods that are often not considered animal products (such as baked goods) are made with cholesterol-containing substances. This information would be important for setting guidelines and informing dietary strategies for disease prevention.

To estimate total and source-specific dietary cholesterol among US adults, we analyzed data on food sources from the National Health and Nutrition Examination Survey (NHANES) – What We Eat in America (WWEIA) 2013–2014. We also described differences in general and specific sources of cholesterol by age, sex, and ethnic group, and compared demographic and dietary patterns for total cholesterol levels. In addition, we documented trends in total cholesterol in seven NHANES-WWEIA datasets from 2001-2014.

The National Institutes of Health and Nutrition Examination Survey (NHANES) is an ongoing research program designed to assess the health and nutritional status of the nonconstitutional civilian population of the United States [8]. NHANES is produced by the National Center for Health Statistics (NCHS) of the Centers for Disease Control and Prevention (CDC). Each year, around 5,000 nationally representative people are selected through a complex, multiple, stratified and close sampling method. The NHANES protocol was reviewed and approved by the NCHS Ethics Committee. All participants provided written informed consent prior to participation.

In our analysis, we used only the first meal recall to maintain continuity across study phases and to compare sample size, as some participants did not complete both meals in the final phase.

The 2013-2014 data set contained 10,175 individual records. After excluding participants younger than 20 years (n = 4406) and those with complete dietary records (n = 722), we included 5047 men and women (20 years or older) who completed the baseline 24-hour dietary questionnaire. our analysis Based on the same eligibility criteria, we included 34,741 participants in the NHANES surveys conducted between 2001 and 2014 for trend analysis.
