Gastroesophageal reflux (GER) is the back-up of food and digestive acids from the stomach into the esophagus (the digestive tube that leads from the mouth to the stomach). Stomach contents are normally held in the stomach by a thick ring of muscle called the lower esophageal sphincter (LES). If the sphincter relaxes, stomach acid backs up into the esophagus, causing irritation and symptoms. Gastroesophageal reflux disease (GERD) is a more severe form of GER that may require treatment.
In children, GER is most often seen in children under 12 months of age when the LES is low. This can lead to frequent fevers, restlessness, and nervousness. Doctors can usually diagnose GERD with a good patient history. Sometimes other tests are needed to make a diagnosis when the history is not so clear. They may include:
Upper GI/barium swallow: This test uses barium, a chalky white liquid that the patient swallows while taking a special series of X-rays. This test is often used to identify other anatomical abnormalities that may explain the symptoms.

Endoscopy: This is an examination performed by a specialist who inserts a small tube with a light camera. The camera can help see abnormalities in the esophagus, stomach, and even the small intestine and help the specialist take tissue samples from abnormal areas.
Gastric emptying test: Sometimes this test is done to determine if there is a delay in gastric emptying, which contributes to GERD. The patient swallows food or drink mixed with radioactive material, which is photographed and detected as it travels through the digestive tract.
PH monitoring: A thin tube is inserted into the esophagus to monitor pH while the patient goes home and records symptoms to see if acid reflux is associated with symptoms.
For children, small, frequent feedings can help reduce GER symptoms. After feeding, keep babies upright for 20-30 minutes. The stomach or left side position may help, but babies should always be monitored to make sure they are breathing easily. Talk to your baby’s pediatrician to see if expressing milk might be a good strategy for managing reflux. Your pediatrician may even prescribe anti-reflux medications (to reduce acid production) for children with more severe GERD symptoms, such as failure to gain weight, extreme fatigue, or respiratory symptoms. Some breastfeeding women with severe GERD may be advised to reduce the amount of milk in the mother’s diet.

Obesity: Losing weight can help reduce symptoms. Many foods that cause weight gain also worsen GERD symptoms. Avoiding GERD triggers can also help patients lose weight.
